类比李某所治"阴火"之证:昔年金元战乱,官员劳倦伤脾,暑热耗气,身热、纳呆、身重、倦怠——此与"饮食劳倦,损伤脾胃,气虚发热"之证何其相似。李某以清暑益气汤,黄芪、人参、白术甘温补中,升麻、柴胡升清阳,麦冬、五味子益气生津,苍术、黄柏清化湿热,中气足而暑热自散。若误用石膏、知母清热泻火,则"寒凉伤中",脾阳更伤,气愈虚而热愈炽,反致倦怠加重、低热缠绵。健脾益气(黄芪、人参、白术、炙甘草)培土益气,升麻、柴胡升清阳,此"甘温除热"之正法。若高热烦渴、舌红苔黄燥、脉洪大,气分实热,可暂佐清热,然中病即止。

从第一性原理——暑为阳邪,其性炎热,六气皆从火化,火热为病之本。此案"低热、纳呆、肢体困重"示暑热夹湿,热为本、脾虚为标。若先健脾益气,甘温之品反助热势、使湿遏热伏;当先清热解暑,使热去湿孤、脾运自健。此非伪两难,乃病机主次之分——清热解暑为先,健脾益气为辅,如王氏清暑益气汤,使热清暑解、气津两复。
